Would you notice the carbon monoxide?

In one sense, you are correct. But the problem is that the cloudiness or odor of burned exhaust gases don't give a clear indication of the CO concentration. If you recognize the burned exhaust gases as a sign of possible CO contamination and take evasive action, you'll be okay. But many people don't recognize the CO concentration because they are still breathing, still functional.
